a. If we are a participating provider in your health plan. We may or may not be in-network
with your insurance company.
b. If you are covered by a State or Federal program with a mandated fee schedule.
c. If you are a member of a Discount Medical Plan Organization. Patients who are
uninsured, or underinsured (limited benefits for medical care), may join a preferred provider
network in our office and will be entitled to network discounts like our insured patients.
d. If you are eligible and choose a payment plan that allows for "Pre Payment" discounts.
e. Patients who meet State or Federal poverty guidelines or other special circumstances
outlined in our "Hardship Policy" may be offered a discount for a period of time as determined
by the clinic. Continuing verification will be required.
4. We may or may not be in network with your insurance company. It is your responsibility to
contact your insurance company and make sure our providers are in network with your
insurance company.
6. It is up to you to contact your insurance company and determine what are your coverage
benefits, copays, deductible, and your percentage payment amount.
7. Your insurance policy is a contract between YOU and YOUR insurance company, NOT the
doctor or clinic and YOUR insurance company. Once your insurance is pre-approved by our
office, as a courtesy to you, we will bill your insurance company directly for covered services
performed. You are personally responsible for any charges not covered by your insurance
company for any reason. It is against State and Federal regulation(s) for us to waive or not
collect your full copay and/or deductible.
8. When quoting benefits your insurance company tells us that, “Quoting Benefits or Coverage is
Not a Guarantee of Payment.” So, we say the same to you: “Our Quoting of Benefits or
Coverage from your insurance Company to you is Not a Guarantee of Payment.” We do not
know what your insurance company will pay for your services until we submit your claim and
are paid. As a courtesy, if your insurance company is approved by us and once your deductible
is met, we will accept your co-pay and percentage payment as a partial patient payment each
visit from you, and then bill the remainder to your insurance company. Should your insurance
company not pay your remainder billed to them or charge back or take back payments made to
the doctor for any reason including refunds or audits then you are responsible for your entire
bill.
9. Our provider may recommend services that are NOT covered by your insurance. You must pay
for these services out of pocket with cash, check or credit card.
